Vital Records
The Vital Records Division issues birth certificates and death certificates from the date of the birth or death until the day before the event turns six (6) months.
Note: Effective January 1, 2018 the fee for each copy of a Birth Certificate or Death Certificate is $25 per each copy regardless of the number of copies ordered at one time. View 2017 and new (effective January 1, 2018) fees (PDF)!
Douglas County Order Form (Within 6 Months of Event Date Occurring)
Certified copies of Birth and Death certificates are only available from the Douglas County Clerk's Office from the date of the birth or death until the day before the event turns six months. When the date from the birth or death reaches six months, we have no access into the State's system to generate the birth or death certificate. If ordering a birth or death certificate within the first six months from the date of the birth or death, please download a Douglas County application/order form from one of the following links:

A completed order form and full payment must be received at time of order. Certified copies are, whenever possible, issued within one working day following receipt of a request. To ensure confidentiality, all applicants are required to show picture identification. Personal checks or money orders are accepted if made payable to: Douglas County Clerk. Visa/Mastercard and Debit cards are also accepted. We will not accept applications without full payment.
State of Oregon Order Form (at 6 Months or Later of Date of Event)
Call Oregon Vital Records at 971-673-1190 or visit their Oregon Health Authority website to apply for a Certified Oregon Birth or Death Certificate if it has been 6 months or more since the birth or death of the individual in the state of Oregon.
