1. What do I do if I get married?

• Notify your department so they can process a personnel action form (PAF) to update your employment records. Be sure to inform your department of any name or address change.
• If you would like to add your new spouse as a dependent on your medical/dental insurance, you must do so within 31 days of the date of the marriage. If not done within 31 days, you must wait until open enrollment for another opportunity to add your spouse. You can add your spouse by logging into your CIS profile and entering an enrollment change. You will need to upload a copy of your marriage certificate.
• Update your PERS beneficiary designations, W4 information and CIS beneficiary designations, if desired.

2. What do I do if I become legally separated or divorced?

• Remove dependent(s) who are no longer eligible from insurance coverage within 31 days of the dissolution of your marriage/legal separation. You can remove dependent(s) by logging into your CIS profile and entering an enrollment change. You will need to upload supporting documentation. Dependent(s) removed from coverage will receive an offer to continue insurance (COBRA) from CIS.
• Update your PERS beneficiary designations, W4 information and CIS beneficiary designations, if desired.

3. What do I do if I have or adopt a baby?

• Add newly eligible dependents to insurance within 31 days of birth (or adoption finalization). If not done within 31 days, you must wait until open enrollment for another opportunity to add newly eligible dependents. You can add new dependents by logging into your CIS profile and entering an enrollment change. You will need to upload supporting documentation.
• Update your PERS beneficiary designations, W4 information and CIS beneficiary designations, if desired.

4. What happens if I have a child who reaches the dependent age limit (age 26)?

Children may remain a dependent for health benefit purposes until the end of the month in which they turn 26. CIS will send a COBRA continuation notice to dependent children approximately one month before their coverage ends. Coverage beyond age 26 is allowed for children who are incapacitated due to a physical, mental or developmental disability that occurred before the child’s 26th birthday. Supporting documentation is required.

5. What do I do if I have a death in the family?

• Check to see if you are enrolled in voluntary life coverage which may apply.
• Update your PERS beneficiary designations, W4 information and CIS beneficiary designations, if desired.

6. What do I do when I retire?

You Need to Do/Know

• Make a counseling appointment with PERS for retirement planning, and if applicable, your deferred compensation plan carrier.
• Give appropriate notice to your supervisor.
• You will receive information from CIS regarding the insurance options available to retirees.

7. What do I do if I leave county employment?

You Need to Do/Know

• Provide at least two-weeks notice to your supervisor/make arrangements for final check
• You may have a number of options for your PERS account
• You and any eligible dependents will receive information from CIS regarding eligibility for COBRA insurance continuation

8. When can I change something in my benefit plans?

• You may change your plan selection once per year during open enrollment, to be effective January 1 of each calendar year.
• Changes are allowed for certain qualifying events (death, divorce, marriage, etc.)

9. What if I move?

• Notify your supervisor of your new address, so they may update your employment records.
• Update your address in your CIS profile

10. What if I change employment status (e.g. from part-time to full-time)?

• If you are moving from a non-benefitted to a benefitted position, you will receive an employee packet regarding insurance options and waiting periods.
• If you are moving from a benefitted to a non-benefitted position, CIS will send you COBRA continuation information.

11. What is Employee Assistance Provider (EAP)?

Through CIS, Douglas County contracts with a third party to provide free, confidential short-term counseling and other services to employees and those within their households. Contact the provider directly (brochures available in the lobby of Human Resources, and as part of your new-employee packet), or email Human Resources staff.
